Introduction / Overview:

 

The Strengthened Quality Standards states that staff must “recognise and respond to deterioration" including physiological, psychological and cognitive changes that indicate a significant worsening of health, condition or wellbeing. Clinical deterioration falls squarely under Standards 3 & 5 but is intertwined across all the standards.  

Key requirements for managing clinical deterioration include:

  • Recognising and responding to changes early

  • Assessment, monitoring and escalation

  • Comprehensive Care Systems

  • Effective Communication

  • Workforce capability

Outline / Objectives:

 

The main objective of this presentation is for staff to recognise, assess, escalate and respond to signs of clinical deterioration, through systematic assessment, advanced clinical judgement, timely communication and evidence-based interventions.


The ultimate goal is individualised, safe, person-centred care.

The presentation will be delivered through evidence-based knowledge, case studies and tools.

  • Identifying those at greater risk of clinical deterioration

  • Early recognition of clinical deterioration - physiological, psychological & cognitive

  • Assessment, critical thinking and monitoring

  • Timely escalation

  • Effective communication

  • Person-centred comprehensive care 

  • Documentation

  • Staff Education

  • Person’s goals of care and Advance Care Planning

  • Understanding palliative care and end-of-life care  

At the beginning and end of every shift each staff member should know who is currently being managed for or at risk of clinical deterioration and the specific care required.
